Examples include Tylenol, or an Aspirin for a headache or a muscle strain. However, some of us have to take medication daily for the rest of our lives due to some physical or systemic disease. There are different categories of medications- Over the Counter (OTC), Prescription, and Herbal are the most common. The OTC medications are the ones that you purchase at your local drug store/pharmacy at your own risk or without a medical provider’s direction.Examples are ibuprofen, Aleve, aspirin, and cough syrup. The prescription drugs are written by a medical provider such as a physician, nurse practitioner, or a physician assistant.Examples include medications to control blood pressure or diabetes.Herbal medications are another category. Some can be prescribed by medical providers such as CoEnzyme Q10 and fish oil supplements. Most natural or herbal substances are not prescribed secondary to inadequate research to be approved by the Food and Drug Administration (FDA).The aforementioned can all be beneficial in one way or another but there can also be untoward effects. It is important to keep in mind that there are different formulations to medications, such as pills, patches, drops, ointments, injections, inhalers, etc.You may ask, how should I or my care giver manage my medications? Below is a short list of key things to be aware of.Make sure there are labels on each medication bottle/package. The label should have the name, dose, maximum number of times indicated per day, when to take the medication, and expiration date just to name a few. Follow the instructions carefully.Know the indication for the medication and the parameters for you not to take it.For example, if you happen to monitor your blood pressure daily and it is significantly below normal, you may have to hold off on taking your blood pressure medication and immediately contact your physician. Taking the medication can lead to dizziness, lightheadedness, and you may even pass out.In the instance that you take medication for diabetes and monitor your sugar levels daily, if it is below normal, do not take the usual dose and immediately contact your prescribing physician. Taking the medication in this case can lead to dizziness, lightheadedness, and you may even pass out.Monitor for side effects or unwanted effects.Examples include nausea, vomiting, headaches, dizziness, loose stools, constipation confusion, and rash to name a few.Keep all medications in a safe place and away from children.Prescription medication should never be shared with another person even if it is your spouse.Keep a current list of all medications. Each time you visit your doctor/hospital, this list should be updated as needed. Inform your medical provider of all medications that you are taking. This list should be readily available in case of an emergency.Discard of the medications that your medical provider has discontinued in order to avoid life threatening events.****Call your medical provider if you are having any problems with your medications, prescription or non-prescription.If a caregiver is managing your medications, then they must be aware of all the above as well.

CAREGIVER FATIGUE / BURNOUT

Taking care of your loved one who is ill is second nature. If it’s for a short while and your loved one progresses to improvement then all seems well with the universe with regard to the sacrifice. There is thanksgiving, happiness, testimony, and rejoicing.
But when there is a decline in functioning, the caregiver can become physically, emotionally, mentally and spiritually exhausted. All the positive energy can turn to negativity and an unconcerned nature.
So how can the caregiver keep abreast of signs of burning out or being stressed to the maximum?
You may ask… What are some of the signs? Some, but not all of the signs are inclusive of: Irritability; loss of interest in usual activities; feeling depressed; withdrawing from the usual company of family and/or friends; decreasing amounts of sleep; feeling of hurting the one you are caring for; falling ill more often.
If some or all of these signs are familiar then what can one do to balance this stressful situation?

Look at every problem as an opportunity. Identify the issues that lead up to this state because as a caregiver you did not start burned out.

Set realistic goals about expectations of your loved one’s specific illness and functionality. Often we want our 80 year old family member to completely recover to their 30 year old self. Research the disease state, treatments(medical and non-medical), and prognosis. Talk to the health care provider(s) and be involved in the treatment plan in order to know what to expect at any given stage.

Ask for help. Even if it’s just you and your loved one in a certain location away from close family, there are community groups, religious groups, social workers or other medical personnel, or support groups specific to your loved one’s illness that are willing to be there. All you have to do is seek and you will find. Some can help, even for a for a few hours.

Set realistic goals with regard to your physical, emotional, and mental limits. Accept what you are humanly capable of doing.

Make time for yourself. Just like you take rest after your 8 hour work day, being a caregiver is also tiring as well. Take a load off once in a while to rejuvenate.

Talk out your frustrations to someone you trust. It can be a therapist, friend, or clergy member. Don’t keep it bottled inside because it will likely blow out of proportion when the stress increases to exponential proportions.

What you put in is what you put out. Eat a healthy diet, exercise, and get a reasonable amount of sleep. You need to remain healthy.

Get back in tune with your loved one and don’t refer to them in your mind as a burden. Treat them as you would want to be treated if the situation were reversed.

Remain positive. Listen to your motivational tapes. Play “Eye of the Tiger” from the Rocky movies. Meditate. Do what you have to do to maintain positive energy. It may be difficult at times but that is life.

These are just a few guidelines. Do not burn out. You need your loved one and your loved one needs you.

MANAGING YOUR NECK AND LOWER BACK PAIN

Have you ever heard the saying…”No Pain, No Gain”. Well that is the good type of pain. Is there such a thing as “good pain”? Well yes, one that yields a successful outcome and gets you closer to your goals.Who am I kidding? The pain that we usually refer to is an unpleasant feeling. Sometimes the pain comes and goes, at times it lingers. Some causes of back pain include trauma(injuries), mechanical(dysfunction of structures of the spine), and acquired/degenerative changes (aging, scoliosis) to name a few.
Structures of the spine that can present as pain generators are inclusive of the bone/joints, disc, nerves, tendons, ligaments, and muscles. Spine pain can also be classified as axial (staying just in the neck or back) vs. radicular(radiating to the arms or legs). Some preventive measures include core strengthening as well as other total body exercises help to stabilize the structures around the spine. A healthy diet also promotes a healthy spine. Illicit substances such as excessive tobacco and alcohol lead to a decline in functioning of the body as well as poor wound healing.
If your pain persists you should seek evaluation from your health care provider. Imaging such as X-rays, MRIs or CT scans can help to delineate the source of pain.
Ways to manage your pain include rest from the particular activity that causes pain. At times a regimented physical therapy or aquatic therapy program is beneficial in restoring biomechanics. Modalities such as heat, ice, bracing as needed can be added to the therapy program. Medications can also help but only under the direction of your health care provider to insure that the analgesics don’t interact with your other medications. If no improvement from the aforementioned, further evaluation by a spine specialist may be required. Interventional spine injections can aide in relief of symptoms. Surgical management is a last resort.

Who says that you can’t continue to enjoy the activities that are pleasurable when you turn the big six zero. Sex was created to be enjoyed into your golden years. There is a concern that certain parts of the body dry up, arthritis sets into the various joints, and that the drive diminishes as one ages but there are ways around that. Intimacy and the yearning is there if you sincerely want to participate. In fact it can be more enjoyable. Just think, you are not a novice at this so have fun with it and experiment in a healthy and safe way.
It is always important to exercise and eat properly in order to maintain an energy level that is sufficient for any activity, inclusive of sex. With the proper stretches and daily activity, it is less likely that you will injure yourself while you and your partner share in the act of intimacy.
Keep the fire burning with all the things you remember doing while you were younger with your partner. Schedule date nights, take a walk down that sensual memory lane, sneak a kiss here and there, hold hands while walking in the park, look into each others eyes for those sparks, tell each other funny jokes, etc. Remember that sex is not just physical, there is a psychological portion to it also. If that feeling is there between the two of you then all with fall into place.
There is outside help when issues beyond your control arise with your body such as lubricants, enhancement medications (Viagara, Cilais), toys to name a few that can prove beneficial. I would advise that you seek advise from your doctor prior to any medications to enhance activity especially if you are on other medications. If you have a cardiac/respiratory history or any illness in which you may become short of breath or reconditioned with increased activity also discuss with your physician because the act of sex can cause exacerbation of the aforementioned.
Monogamy is best, but it is important to utilize protection with a physical barrier such as a condom for safety against sexually transmitted diseases.
Go for it. Be safe. Have fun. You are not 20. You are 60 + and fabulous.
